This position plays a critical role in managing referrals, coordinating admissions, and serving as a key communication link between hospitals, providers, families, and our interdisciplinary care team.QualificationsPrevious experience in healthcare admissions, intake coordination, case management, or skilled nursing preferredKnowledge of Medicare, Medicaid, managed care, and insurance authorization processes preferredStrong communication and interpersonal skillsExcellent organizational and multitasking abilitiesAbility to work efficiently in a fast-paced environmentProficiency with electronic medical records and Microsoft OfficeHigh school diploma or equivalent required; healthcare-related education preferredResponsibilitiesManage and coordinate incoming referrals and admissions for the skilled nursing facilityReview referral documentation for completeness, insurance coverage, and clinical appropriatenessCommunicate with hospitals, case managers, physicians, families, and internal departments regarding admission status and bed availabilityCollaborate with nursing leadership and clinical staff to facilitate smooth patient transitionsVerify insurance benefits and obtain required authorizations when applicableMaintain accurate records in electronic medical record systems and admission tracking toolsCoordinate transportation and admission logistics as neededEnsure compliance with facility policies, HIPAA regulations, and payer requirementsSupport census development and customer service initiativesWhy Join RivertownWe believe in supporting the people who care for our residents every day.
